If you are an individual residing in the United States who received more than one text message from June 12, 2021, through June 16, 2026 from Medical Management of Chicago, LLC d/b/a USA Clinics Group, after opting out or while your number was listed on the National Do Not Call Registry (“DNC”), you may be entitled to a payment from a Class Action Settlement. Excluded from the Settlement Class are all class members who timely and validly exclude themselves from the Settlement.
If you are a Settlement Class Member and wish to receive payment then your must submit a Claim Form by October 5, 2026.
USA Clinics Group will establish a Settlement Fund up to $574,350.00. The Settlement Fund will be used to pay notice and administration costs, attorneys’ fees and costs awarded to Class Counsel, and any service award approved for the class representative. The net remaining funds will be used to pay Settlement Class Members who submit valid claim forms for the benefit described below.
Settlement Class Members who submit a timely, valid, complete, and verified Claim Form will receive a payment of up to $50.00 per text message with a cap of three messages per person. If the Settlement Fund is insufficient to pay $50.00 for each text message for all approved claims, the per-message amount will be uniformly reduced on a pro rata basis so that the total of all payments, together with any Court-approved attorneys’ fees and expenses, notice and administration costs, and any service award, does not exceed the Settlement Fund.

The deadline to submit a Claim Form online is October 5, 2026. If you are mailing your Claim Form, it must be mailed with a postmark date no later than October 5, 2026.
